55 BOTTLES OF 5-HOUR ENERGY SWEEPSTAKES
1. NO PURCHASE NECESSARY
2. HOW TO ENTER: Individuals may enter the sweepstakes by completing all required contact information on a survey card at the 5-Hour Energy booth at Great West Truck show in Las Vegas, Nevada (June 20 – 22, 2009) or at the Great American Truck Show in Dallas, Texas (August 20 – 22, 2009) and by returning the card to the 5-Hour Energy booth at that particular show. The survey card will also ask optional questions regarding 5-Hour Energy under the heading “We Value Your Opinion.” Completion of these questions is not required to enter the sweepstakes.
Individuals may enter this sweepstakes by filling out the form only once. More than one entry from any person will void all entries from that person.
Survey cards may be submitted to the 5-Hour Energy Booth at the Great West Truck Show in Las Vegas, Nevada from June 20 – 22, 2009 and at the Great American Truck Show in Dallas, Texas from August 20 – 22, 2009. The contest closes upon the conclusion of the Great American Truck show in Dallas, Texas on August 22, 2009.
Sponsor is not responsible for technical failures in entry submission, or lost, late, misdirected, damaged, incomplete, or illegible survey cards relating to the promotion.
3. ELIGIBLITY: The sweepstakes is open only to legal residents of the 50 United States and D.C., 18 years of age and older who are registered attendees of the Great West Truck show in Las Vegas, Nevada (June 20 – 22, 2009) and the Great American Truck Show in Dallas, Texas (August 20 – 22, 2009). Employees of Living Essentials, its advertising and promotion agencies, and their immediate family members and/or those living in same household are not eligible.
4. DETERMINATION: On or about September 1, 2009, Sponsor will randomly select one winner from all eligible entries received. Odds of winning depend on number of eligible entries received. The Sponsor’s decisions are final and binding in all matters relating to the sweepstakes.
On or about September 4, 2009, the winner will be notified by phone, mail and/or e-mail at the location provided on the survey card. In the event that the winner cannot be notified, fails to respond to the notification within the time period allowed by the notification, or the notification is returned as undeliverable, they will be disqualified and another survey card will be drawn at random.
5. PRIZE: Fifty Five (55) bottles of 5 Hour Energy liquid energy shots (“Prize”) will be awarded to the winners. Approximate cash value of the Prize: $148.00. The Prize is not transferable, cannot be redeemed for cash and must be accepted as awarded. In the event the Prize cannot be awarded as stated, Living Essentials may substitute a prize of equal or greater value in its sole discretion. The winners may be asked to execute an Affidavit of Eligibility/Release of Liability and Publicity within a reasonable time period, which time period shall be specified in the winner’s notification, certifying that the entry is the winner’s. Non-compliance within this time period may result in disqualification and another entrant will be drawn and selected as the winner. All entries are subject to verification.
By accepting a prize, the winner agrees to release Sponsor and its parent companies, subsidiaries, affiliates, related companies and advertising and promotion agencies, and each of their respective officers, directors, employees and agents from any and all liability, loss or damage of any kind arising out of or in connection with their participation in this contest or with respect to awarding, receipt, possession, use and/or misuse of the Prize. Sponsor is not responsible for any typographical or other errors in the printing, the offering or the announcement of any prize or the promotion.
All federal, state and local taxes are the sole responsibility of the winners. All federal, state and local laws and regulations apply.
6. INDEMNIFICATION. By participating, participants agree to release, discharge, indemnify and hold harmless Sponsor and each of its respective officers, directors, employees, representatives, agents, advertising and promotional agencies from and against any claims made by winners, entrants, or any other third parties, related in any way to the operation of this sweepstakes as well as any other claims, damages or liability due to any injuries, damages or losses to any person (including death) or property of any kind resulting in whole or in part, directly or indirectly, from acceptance, possession, misuse or use of any prize or participation in any sweepstakes-related activity or participation in this sweepstakes.
7. GENERAL RULES: By submitting an entry, all entrants agree to be bound by these official rules in all matters related to this contest. All federal, state, and local taxes are sole responsibility of winners. All federal, state, and local laws and regulations apply. Void where prohibited. By submitting an entry, entrants grant permission to Living Essentials and its agencies to use the winner’s name and/or likeness for purposes of advertising/trade, without further compensation to entrants unless prohibited by law. If any provision of these Official Rules is found to be invalid or unenforceable by a court of competent jurisdiction or appointed arbitrator, such determination shall in no way affect the validity or enforceability of any other provision therein. Any claims relating to this contest shall be governed by Michigan law without regard to its conflict of law rules, and any such claim must be heard in Michigan.

To enter complete and return the survey card found at the 5-Hour Energy booth at the Great West or Great American Truck Shows. Must include all necessary contact information, including name, address, and phone number, as well as an e-mail address. Submit your entry at the 5-Hour Energy booth at the Great West or Great American Truck Shows. All Entries must be submitted on or before August 22, 2009. Void where prohibited.
